Service Requests This form serves as an initial request for on-site videography and/or photography services for VIP Elite Members of CDMA Business Model LLC. Submission of this request does not guarantee service availability, scheduling, or approval. 2. Availability & Scheduling All requests are subject to review, approval, and scheduling availability. We recommend submitting requests at least 30 days in advance of the desired event date. Priority may be given to pre-approved and contracted events. 3. Additional Terms & Conditions Upon review of your request, additional terms and conditions, including but not limited to location requirements, release forms, talent agreements, licensing arrangements, and equipment specifications, may apply. These will be provided prior to final approval. 4. No Automatic Partnership or Sponsorship Submission of this form does not imply any partnership, sponsorship, endorsement, or ongoing relationship between the requestor and CDMA Business Model LLC. 5. Fees & Payment Any associated fees will be outlined in a separate agreement upon acceptance of the request. Payment terms will be provided and must be met prior to service delivery. 6. Intellectual Property All photographs, video footage, and related media captured during the approved service are the exclusive property of CDMA Business Model LLC, unless otherwise agreed in writing. 7. Prohibited Misrepresentation You may not falsely advertise, misrepresent, or imply any partnership, sponsorship, or affiliation with CDMA Business Model LLC by submitting this form or by use of our name, logo, or branding without written consent. 8. Jurisdiction & Dispute Resolution These terms shall be governed by and construed under the laws of the State of New York. Any disputes shall be resolved through binding arbitration in New York, NY, administered by CDMA Business Model LLC. 9.
